Job summary

Graceville Rehab is seeking a dedicated Registered Nurse to support CNAs and collaborate with the IDT to deliver resident-centered care in a faith-based, compassionate setting. You will conduct assessments, participate in physician rounds, ensure timely documentation, and coordinate referrals while upholding clinical standards and facility policies to promote resident well-being.

Qualifications

• Active RN license in good standing in the state of practice.
• Experience in a skilled nursing or long-term care setting is preferred but not required.

Responsibilities

• Supervise and support CNAs under the direction of the Director of Nursing.
• Collaborate with the interdisciplinary team (IDT) to assess, plan, implement, and evaluate individualized care plans.
• Ensure initial baseline and periodic comprehensive assessments are completed on time.
• Prepare for and facilitate physician rounds by flagging concerns and preparing documentation for review.
• Initiate consultations or referrals as appropriate based on resident needs.
• Perform administrative tasks including medical documentation, evaluations, and reports.
• Deliver nursing care within scope of practice and aligned with facility policy and clinical standards.
• Monitor residents for changes in condition (e.g., confusion, fever, breathing difficulty, pain, appetite changes, mobility loss) and notify providers as needed.
• Perform other duties as assigned by the Director of Nursing
