Job summary

Access Home Health in Foley, AL is seeking a Registered Nurse to join our Home Health team on a PRN basis. The RN provides and directs nursing care in patients' homes under physician orders, coordinating care with family and providers.

Requirements include 1 year home health or hospice experience, current RN license in Alabama, CPR certification, valid driver's license and access to a vehicle, and ability to travel to multiple patient residences daily.

Qualifications

• One year of Home Health or Hospice RN experience.
• Current RN license in the state where services are rendered (Alabama).
• Current CPR certification, driver's license, vehicle insurance, and access to a dependable vehicle.
• Must be able to reliably commute to multiple patient residences daily within the geographic area.

Responsibilities

• Create, implement, execute and coordinate the plan of care to promote wellness and prevent rehospitalization in patients' homes.
• Maintain documentation and clinical notes with patient condition, goals, interventions and responses.
• Coordinate with physician, agency personnel, patients, families and resources to ensure continuity of care.
• Provide high quality clinical home health services within the scope of practice from admit through discharge.

A few position highlights include:Create, implement, execute, and coordinate the overall plan of care to promote wellness and prevent re-hospitalization for patients in their homes.
