Job summary

Qualifications

Responsibilities

• Create, implement, and coordinate the overall plan of care to promote wellness and prevent rehospitalization for patients in their homes.
• Perform initial and comprehensive nursing evaluation visits and develop the plan of care within state guidelines.
• Maintain and submit accurate documentation and clinical notes throughout care.
• Establish and maintain an efficient visit route for patient visits.
• Provide high quality clinical nursing services within the plan of care from admit through discharge.
