Job summary

Apploi is seeking a dedicated Registered Dietitian to join our interdisciplinary care team for a full-time position split between Bradford Manor Nursing and Rehab in Bradford, PA, and Kinzua Nursing and Rehab in Warren, PA. The RD will support nutritional care for residents in a skilled nursing setting.

The ideal candidate holds an RDN credential, PA licensure, and at least 1 year of clinical experience in SNF/long-term care, with strong communication and documentation skills.

Qualifications

• Bachelor’s degree in Nutrition, Dietetics, or related field.
• Registered Dietitian Nutritionist (RDN) credential required.
• Licensure in the state of Pennsylvania
• Minimum of 1 year of clinical experience, preferably in SNF/long-term care
• Strong communication, organizational, and clinical documentation skills
• Reliable transportation for travel between locations

Responsibilities

• Provide nutritional assessments, care planning, and ongoing evaluation of residents' nutritional needs.
• Develop and implement individualized nutrition plans in coordination with nursing and medical staff.
• Monitor residents' clinical status and adjust nutrition plans as needed.
• Participate in care plan meetings and collaborate with the interdisciplinary team.
• Ensure compliance with federal, state, and local regulations, as well as facility policies.
• Educate staff, residents, and families on nutrition-related topics.
• Conduct regular audits of dietary services and assist with menu planning and review.
• Travel between the Bradford and Kinzua facilities as scheduled.
