Apploi in Bradford, PA and Warren, PA is seeking a Registered Dietitian to join our interdisciplinary care team in a full-time role. The position is split between Bradford Manor Nursing and Rehab and Kinzua Nursing and Rehab and serves residents in a skilled nursing facility setting.

You will conduct nutritional assessments, develop care plans, coordinate with nursing and medical staff, monitor clinical status, educate staff and families, and travel between sites as scheduled.

Qualifications

• Bachelor's degree in Nutrition or Dietetics.
• RDN credential required.
• Pennsylvania licensure required.
• Minimum 1 year of clinical experience, preferably in SNF or long-term care.
• Strong communication, organizational, and clinical documentation skills.
• Reliable transportation for travel between locations.

Responsabilités

• Provide nutritional assessments, care planning, and ongoing evaluation of residents' nutritional needs.
• Develop and implement individualized nutrition plans in coordination with nursing and medical staff.
• Monitor residents' clinical status and adjust nutrition plans as needed.
• Participate in care plan meetings and collaborate with the interdisciplinary team.
• Ensure compliance with federal, state, and local regulations, as well as facility policies.
• Educate staff, residents, and families on nutrition-related topics.
• Conduct regular audits of dietary services and assist with menu planning and review.
• Travel between the Bradford and Kinzua facilities as scheduled.
