Job summary

Qualifications

• Current RD or RDN credential required.
• Active Pennsylvania LDN license required.

• Knowledge of CMS and Pennsylvania healthcare regulations preferred.
• Strong communication, organizational, and documentation skills.
• Ability to work independently and with interdisciplinary team.

Responsibilities

• Complete nutritional assessments and reassessments.
• Develop individualized nutrition recommendations, interventions, and follow-up plans.
• Monitor resident weights, dietary intake, hydration, and nutritional concerns.
• Collaborate with interdisciplinary teams to develop and update resident care plans.
• Provide nutrition input for MDS assessments and care planning as applicable.
• Review menus, therapeutic diets, supplements, and meal accommodations to ensure nutritional adequacy.
• Support residents with specialized dietary needs, including modified textures and enteral nutrition.
• Provide nutrition education to residents, families, and staff.
• Work with dining services to promote food quality and resident satisfaction.
• Maintain documentation in electronic medical records.
• Assist with quality improvement initiatives, regulatory surveys, and compliance activities.
• Provide nutrition recommendations and dining strategies tailored to individuals living with dementia.

Key Responsibilities

• Complete nutritional assessments and reassessments, identifying residents at risk for nutritional deficiencies, weight changes, or dehydration.
• Develop individualized nutrition recommendations, interventions, and follow-up plans.

• Previous experience in long-term care, skilled nursing, or senior living preferred.
• Knowledge of therapeutic diets, nutritional assessments, and individualized care planning.
• Familiarity with CMS and Pennsylvania healthcare regulations preferred.
• Strong communication, organizational, and documentation skills.
• Ability to work independently while collaborating with an interdisciplinary team.
• Experience with electronic medical records preferred.
