Job summary

UF Health seeks a Nurse Navigator to serve as the vascular patients central point of contact throughout the surgical care continuum. The role coordinates multidisciplinary care across UF Health NE Florida regional hospitals and works with physicians to develop care pathways and monitor outcomes.

The RN Navigator Lead will supervise 3–4 navigators, coordinate testing, imaging and referrals, and engage in quality improvement initiatives across four NE Florida sites.

Qualifications

• 5 years nursing experience required.
• 3 years vascular surgery nursing preferred.

Responsibilities

• Coordinate multidisciplinary care across internal and external providers.
• Serve as the central point of contact for vascular surgery patients.
• Develop and maintain clinical protocols and care pathways.
• Track program metrics and patient outcomes with administration.
• Lead navigator team and supervise 3–4 RN Navigators.
