Job summary

Nuvance Health Med Practice CT is seeking a dedicated Registered Nurse Navigator to coordinate the care of patients with vascular conditions. You will work across pre-procedure, hospitalization, discharge, and follow-up phases to ensure a seamless patient experience.

The role requires a BSN (preferred), an active RN license, BLS (ACLS preferred), and a minimum of three years in clinical nursing with vascular surgery exposure.

Qualifications

• Graduate of an accredited school of nursing.
• Bachelor of Science in Nursing (BSN) preferred.
• Current Registered Nurse (RN) license in the state of practice.
• Current BLS certification required.
• ACLS certification preferred.
• Minimum of 3 years of clinical nursing experience.
• Experience in vascular surgery and EMR systems preferred.
• Prior patient navigation, care coordination, or case management experience preferred.

Responsibilities

• Patient Navigation—serves as the main contact point, guiding patients through the continuum of care (pre-procedure, hospitalization, discharge, and follow-up).
• Clinical Coordination—coordinates with physicians and specialists for complex aneurysm/dissection cases, including surveillance programs.
• Education and Advocacy—educates patients/families on treatment options, disease processes, and self-care.
• Operational Support—tracks patients, reduces readmissions, and ensures adherence to follow-up.
• Clinical Triage—acts as liaison for clinical questions, triages patient calls and adjusts medications under physician direction.
• Quality Improvement—collaborates with the team to maintain patient registries and track outcomes for program growth.
• Maintains and models Nuvance Health values.
• Demonstrates regular, reliable and predictable attendance.
• Performs other duties as required.
