UF Health seeks a Nurse Navigator to coordinate care for the surgical patient population across the care continuum. You will ensure delivery of needed services, help patients overcome barriers, and build trusting relationships with patients, families, and caregivers.

The role supports a multidisciplinary team, tracks outcomes, maintains clinical protocols, and provides education to patients and the healthcare team. Strong communication and BLS are required.

Qualifications

• 3 years nursing or nurse navigator experience preferred.
• Basic Life Support (BLS) required at time of hire.

Responsabilités

• Promotes patient- and family-centered care.
• Tracks metrics and patient outcomes with administration.
• Coordinates entry into the EMR and care pathways.
• Facilitates referrals and psychosocial support for patients and families.
• Maintains communication with all health team members and patients.
• Works with cancer committee and admins to identify service gaps and quality improvements.
• Educates patients, families, and caregivers about diagnosis and plan of care.
