University of Florida Jacksonville Physicians, Inc. is seeking a Nurse Navigator to coordinate care for surgical patients across the care continuum. The role involves advocating for patient-centered care, ensuring timely access to services, and collaborating with physicians and the healthcare team.

The position supports tracking outcomes, maintaining care pathways, and serving as the central contact for patients and families, including facilitating referrals during high-stress periods.

Formación

• 3 years nursing or as a nurse navigator or similar role.
• High School Diploma or GED required.
• Associates Nursing required; Bachelors Nursing preferred.
• Basic Life Support (BLS) required at time of hire.
• Travel up to 10%.

Responsabilidades

• Promotes a patient- and family-centered care environment for ethical decision making.
• Advocates for patients to promote optimal care and outcomes.
• Participates in tracking metrics and patient outcomes with administration.
• Develops and maintains clinical protocols/care pathways and EMR entry.
• Coordinates care across multidisciplinary teams and acts as central contact.
• Tracks patient care scheduling and follow-up, and supports referrals.
