Job summary

UF Health seeks a Nurse Navigator to ensure care coordination for the surgical patient population across the continuum. You will work with the multidisciplinary team to remove barriers, support patients and families, and deliver a safe, timely, and patient-centered care experience.

Minimum 3 years nursing or nurse navigator experience is preferred; travel up to 10% is required. UF Health emphasizes equal opportunity and a drug-free workplace.

Qualifications

• Minimum 3 years nursing or nurse navigator experience.
• BLS certification required at time of hire.
• Associate's nursing degree or higher required.
• Bachelor's degree in nursing preferred.

Responsibilities

• Promotes patient- and family-centered care and advocates for optimal outcomes.
• Tracks patient outcomes and reports findings to the cancer committee.
• Coordinates entry into the EMR and maintains clinical protocols/care pathways.
• Collaborates with physicians and health professionals to coordinate the care plan.
• Ensures timely access to psychosocial support and appropriate referrals.
• Maintains open communication with health team and coordinates tests/procedures.
• Participates in quality improvement and community partnerships.
• Provides care education to patients, families, and healthcare team members.
• Educates referring physicians and the community about services.
• Tracks patients in the Surgical Home Program and follow-up scheduling.
