UF Health in Florida seeks a Nurse Navigator to coordinate care for surgical patients across the care continuum; you will remove barriers to care and build trusting relationships with patients, families, and providers.

You will work with the care team to track metrics, educate patients, and coordinate entry into the EMR, ensuring timely access to psychosocial support and appropriate referrals.

The role requires nursing experience, BLS certification, and willingness to travel up to 10%.

Formación

• 3 years nursing or as a nurse navigator or similar role preferred
• Associates Nursing required
• Bachelors Nursing preferred
• Basic Life Support (BLS) certification required at time of hire
• Travel up to 10%

Responsabilidades

• Promotes a patient- and family-centered care environment for ethical decision making.
• Advocates for patients to promote optimal care and outcomes.
• Tracks metrics and outcomes and reports findings to the cancer committee.
• Coordinates entry of care plans into the EMR with physicians and staff.
• Facilitates referrals for psychosocial support during high-stress periods.
• Maintains open communication with health team and with patients and families.
