Lake Forest Senior Living at Mountain Home is seeking a dedicated RN Assessment Coordinator (RNAC) to join our regional team as a full-time Regional MDS Coordinator. You will oversee seven long-term care campuses across four states, ensuring accuracy in MDS/RAI processes and regulatory compliance.

• Active RN license is required.
• Experience in long-term care is essential.
• RAC-CT certification preferred.
• Strong knowledge of MDS and RAI processes.
• Excellent organizational and time-management skills.
• Effective communication and interpersonal skills.
• Proficient in relevant computer applications.
• Strong knowledge of Quality Measures/Quality Indicators.

Responsibilities

• Conduct comprehensive assessments of residents' health status and care needs.
• Coordinate with interdisciplinary teams to develop individualized care plans.
• Ensure accurate and timely completion of the MDS and RAI processes.
• Collaborate with healthcare providers to implement and monitor care plans.
• Stay current with regulatory changes related to MDS/RAI.
• Provide training and support to staff on MDS and RAI processes.
• Communicate effectively with residents, families, and healthcare providers.
• Monitor case mix index and quality measures.
