Develop and maintain high-level relationships with hospitals, health systems, physicians, case managers, and referral sources across Kansas.
Support census growth and market development through strategic referral partnerships.
Mentor and coach Patient Care Liaisons with ride-alongs and field visits.
Collaborate with Clinical Leadership and Facility Administrators to optimize discharge communications.
