Job summary

Catholic Charities in Houston is seeking a Case Manager I to support refugee resettlement efforts. You will conduct intake, assess eligibility, and develop individualized service plans to help clients achieve self-sufficiency and stability.

You will maintain comprehensive case notes, manage documentation in Client Track and Refugee Management Solution, and coordinate referrals and outreach activities, including airport pickups and volunteer training as needed.

Qualifications

• High School Diploma or GED required.
• One year of experience in an administrative setting or with refugee populations preferred.
• Valid Texas driver's license required.

Responsibilities

• Assess client eligibility for program services, complete intakes, and create individualized service plans.
• Provide services identified on service plan in a timely manner.
• Make necessary referrals within and outside the agency as needed.
• Maintain client files, update case notes, and track activities in Client Track and Refugee Management Solution.
• Assist with outreach, training volunteers and interns, and airport pickups as needed.

The Case Manager I is responsible for working with clients participating in the refugee resettlement programs. The position is responsible for intake, assessment, and providing quality case management services for all assigned cases. The successful candidate will display dignity and respect toward both potential clients and clients already enrolled in the program, show initiative while balancing multiple responsibilities, and be able to work independently while following up on assigned cases.# **PRINCIPAL DUTIES*** Assess client eligibility for program services, complete intakes, and create individualized service plans. Provide services identified on service plan in a timely manner. Make necessary referrals within the agency and outside of the agency, as needed. Ensure that services are provided in accordance with LEP policy.* Maintain client files, update case notes, and update calendar.
