World Relief seeks a Family Case Management Case Associate to support refugees and immigrants in Chicago in accessing healthcare and wellness services. You will coordinate transportation, case management, and connections to English classes and community resources with World Relief staff and local partners.

Qualifikationen

• Bachelor’s degree in nursing, pre‑medicine, social work, counseling, or related field is desired.
• Ability to use MS Office suite for documentation and communication.
• Fluency in Spanish or other languages is preferred.

Aufgaben

• Provide safe, reliable driving for group activities, outings, and events.
• Assist clients in identifying and accessing health care resources and Medicaid options.
• Provide case management including scheduling appointments, medication management education, and transportation planning.
• Refer participants to external services and coordinate with partners and local churches.
• Maintain confidential, accurate case files in accordance with grant requirements.
• Support group activities and community building through outreach and follow‑up.
