World Relief seeks a Family Case Management Group Admin & Case Associate to support wellness groups and case management for clients with high medical needs in Chicago. This role increases community connections and reduces social isolation, coordinating with staff, partners, and local churches.

The position covers RHP program activities, including testing, attendance tracking, and data entry, with a caseload of about 10 high-needs clients, involving medical appointment support and family training

Qualifications

• Mature Christian faith and alignment with World Relief values.
• MS Office proficiency and ability to enter data accurately.
• Strong cross-cultural sensitivity and mediation skills.
• Excellent organizational skills and detailed record-keeping.
• Experience with case management for vulnerable populations preferred.

Responsabilités

• Provide administrative support for RHP groups, including pre- and post-tests, attendance tracking, and data entry; coordinate transportation and supplies.
• Assist approximately 10 high-needs clients under case managers’ direction.
• Help clients without Medicaid identify health care resources and financial assistance options.
• Conduct or assist with research to find providers; schedule appointments; educate on prescription management and records.
• Refer refugees to external services and ensure access; maintain confidential case files per grant rules.
• Participate in department meetings, trainings and organizational events; collaborate with partners.
