Lutheran Social Services of Wisconsin & Upper Michigan in Milwaukee seeks a Care Coordinator for our Refugee Resettlement Program. You will develop individualized service plans, assess client needs, build employer partnerships, and lead employment readiness workshops to help clients enter the workforce.

You will coordinate services with community resources, document progress, and support clients through onboarding, resumes, and applications, while collaborating with interpreters and providers

Responsibilities

• Develop individualized service plans to help refugee clients achieve employment goals.
• Assess client needs, strengths, barriers, and opportunities for successful employment.
• Build and maintain relationships with local employers to identify job opportunities.
• Conduct weekly Employment Workshops on workplace expectations, readiness skills, and culture.
• Coordinate services and connect clients with community resources and supports.
• Collaborate with refugee families, interpreters, and service providers.
• Facilitate client meetings and advocate for client success.
• Assist with employment onboarding paperwork and requirements.
• Coordinate orientation dates with employers and arrange interpretation as needed.
• Develop resumes, share job leads, and help with applications.
• Maintain documentation and prepare reports for funders.
• Work a flexible schedule including occasional evenings and weekends.
• Report data into funders' databases.
