Responsibilities

• Contact PCP offices to obtain updated referrals.
• Document referral details in the EHR and PM system.
• Monitor expiration dates and remaining visits for coverage.
• Maintain referral queues and upcoming appointment lists.
• Escalate unresolved referral issues when needed.
