Job summary

Adventist Health Hanford is seeking a dedicated referrals coordinator to manage incoming and outgoing referrals, schedule appointments, and maintain strong relationships with referring providers.

You will verify insurance benefits and authorizations, assist patients and families, and coordinate with the Care Connection Center to ensure timely care and effective communication across teams.

Qualifications

• High School Education/GED or equivalent (preferred).
• Associate’s/Technical Degree or equivalent combination of education/related experience (preferred).

Responsibilities

• Processes inbound and outbound referrals and verifies referrals per protocols in the EHR.
• Obtains benefit verification and necessary authorizations prior to patient arrival for ambulatory visits and radiology services.
• Verifies patient demographics and insurance eligibility including coordination of benefits.
• Acts as navigator/contact for patients and families, addressing questions and concerns.
• Collaborates with Care Connection Center and responds to provider and staff questions about referrals and care coordination.
• Performs other job-related duties as assigned.
