Ohio's Hospice, Inc. is seeking a Patient & Referral Services Representative in Dayton, OH. You will be the first point of contact for patients, families, referral sources, and care teams across our service areas. The role emphasizes compassionate communication and efficient data handling in a fast-paced environment.

The position offers full-time hours (40 per week) with shifts between 12:00 p.m. and 10:30 p.m., including every third weekend and 1-2 holidays per year.

Qualifications

• High school diploma or GED required.
• Strong computer and data-entry skills.
• Excellent communication and customer-service skills.
• Valid driver's license with a clean driving record.
• Ability to travel in varying weather conditions.
• Criminal background check and 10-panel drug screen required.
• Two years of customer-service experience preferred.

Responsibilities

• Answer incoming calls and inquiries with professionalism, empathy, and exceptional customer service.
• Gather, verify, and document patient and referral information.
• Explain hospice services clearly and answer questions from patients, families, providers, and partners.
• Maintain accurate, complete, timely documentation of all interactions.
• Collaborate with team members and support other departments as needed.
• Participate in quality-improvement initiatives to enhance patient and family experience.
• Assist with onboarding and training new team members when assigned.
