Job summary

Ohio's Hospice in Columbus is seeking a dedicated Personal Care Specialist to provide hands-on support to patients receiving hospice services in home and facility settings.

You will work with patients, families, and the care team to promote comfort, independence, and dignity, with travel across the Columbus service area and a regular weekday schedule.

Qualifications

  • High school diploma or GED is required.
  • CHPNA certification is a plus; encouraged to pursue hospice certification.
  • Comfortable documenting patient care in EMR.
  • Valid Ohio driver's license and good driving history.

Responsibilities

  • Provide hands-on personal care to hospice patients and families.
  • Collaborate with care team to promote comfort, independence, and dignity.

Ohio's Hospice is seeking a caring, dependable Personal Care Specialist to provide hands-on support to patients receiving hospice services. In this meaningful role, you'll work closely with patients, families, and members of the hospice care team to promote comfort, independence, and dignity throughout each patient's care journey.

This is a field-based position, with visits taking place in patients' homes and care facilities throughout the Columbus service area.

  • Schedule: Monday-Friday, 8:30 a.m.-4:30 p.m.
  • Holiday Coverage: Two required holidays per year as part of the rotating holiday schedule
  • Service Area: Marysville, Mt. Gilead, and Columbus, Ohio

Because this position involves travel throughout the service area, reliable transportation and a valid Ohio driver's license are required.

What You'll Bring
  • High school diploma or GED
  • CHPNA certification is a plus. Ohio's Hospice encourages and supports eligible employees interested in pursuing hospice and palliative care certification.
  • Comfortable using computers and electronic medical record systems to accurately document patient care
  • Valid Ohio driver's license, good driving history, and current automobile insurance
  • Ability to successfully complete required background screening and 10-panel drug screening
