DAP Health is seeking a Referral Care Coordinator to join our clinical team in California. The role ensures continuity of care by coordinating timely specialty or ancillary appointments and managing referral data entry and documentation within the EHR system.

The ideal candidate will have at least 1 year of medical field experience, CPR/First Aid certification, and bilingual abilities in Arabic/Farsi. Strong communication and organizational skills are essential to support patients and providers

Competenze

• Diploma from an approved High School or GED Equivalent.
• One year of medical field experience with understanding of insurance and medical terminology.
• CPR and First Aid certification must be current and on file.
• Bilingual in Arabic/Farsi required.
• Excellent oral and written communication skills and interpersonal abilities.

Mansioni

• Process referrals according to urgency and keep the referral tracking system up to date.
• Data entry of new referrals into EHR/i2i referral module and document all interventions.
• Submit insurance authorizations and track approval status; record procedure numbers and authorization periods.
• Coordinate between patient, PCP, and specialty provider to schedule appointments.
• Notify patients and PCPs of delays or barriers to care; re-schedule as needed.
• Maintain reports, ensure timely receipt of all referral-related documents, and close referrals when reports are received.
• Collaborate with Care Coordinator Specialist to identify patients for program enrollment and assist with follow-up.
