Job summary

Northern Counties Health Care, Inc. is seeking a Referral Coordinator to support timely access to specialty care and other services.

The role coordinates, processes, tracks, and follows up on patient referrals while working with patients, clinical staff, specialty offices, insurers, and community partners. The position may provide coverage for medical reception duties as needed, ensuring referrals are complete, properly authorized, scheduled, and documented in the electronic health record.

Qualifications

• High school diploma or equivalent is required.
• Experience in a medical office, referral, scheduling, patient access, or healthcare administrative role preferred.
• Experience with electronic health records and insurance authorization processes preferred.
• Knowledge of medical terminology preferred.
• Strong organizational skills and ability to manage a high volume of detailed, time-sensitive work.
• Strong written, verbal, and interpersonal communication skills.
• Ability to maintain confidentiality and exercise sound judgment.
• Ability to work independently while collaborating with a multidisciplinary team.

Responsibilities

• Coordinate incoming and outgoing referrals within established timeframes.
• Review referral requests for completeness and obtain missing information.
• Coordinate with providers, staff, patients, and offices to facilitate referral completion.
• Determine whether prior authorization, insurance approval, or additional documentation is required and complete steps.
• Send required records and supporting documentation securely to receiving organization.
• Assist patients with referral questions, scheduling, and next steps.
• Track referrals from initiation through completion and follow up on pending referrals.
• Obtain and route consultation notes, test results, and other documentation following a referral.
• Maintain accurate referral information in the electronic health record and other systems.
• Identify referral barriers and communicate concerns to the provider or patient.
• Prioritize urgent and time-sensitive referrals per procedures.
• Maintain knowledge of referral requirements and community resources.
• Support referral work queues and reports used to monitor access and closure.
• Protect patient confidentiality and handle protected health information.
