Qualifications

• High School diploma/GED required.
• Experience accessing and navigating community resources preferred.
• Ability to manage caseload and handle conflicts effectively.
• Excellent verbal and written communication skills in a customer service setting.
• Ability to adapt during transition periods and take on additional tasks as needed.
• Strong problem-solving and interpersonal skills.

Responsibilities

• Identify and prioritize members' health care goals with the care team.
• Facilitate access to medical, mental health, and social services for members and dependents.
• Liaise with city agencies and community organizations to support clients.
• Document all encounters and follow-up plans in Epic.
• Coordinate meetings with the health services team and community partners.
• Assist members with scheduling medical and social services appointments and transportation.
• Participate in case conferences and provide status updates.
