Job summary

Sacramento Native American Health Center is seeking a Referral Coordinator to support the medical department by organizing internal and external referrals, ensuring that appointments, required documentation, insurance information and prior authorizations are properly established.

The role may float to other SNAHC locations and requires strong computer skills, excellent communication, confidentiality, and the ability to work independently in a fast-paced healthcare setting.

Qualifications

• High School diploma or equivalent required.
• Minimum 1 year of experience in an office setting.
• Excellent computer and technology skills.
• Ability to communicate orally and in writing; professional demeanor and the ability to represent SNAHC in the community.
• Must maintain a high degree of confidentiality.

Responsibilities

• Receives referral requests from providers and communicates with them or the patient’s care team as needed.
• Obtains prior authorizations for referrals to specialists and surgeries; notifies if authorization is denied.
• Verifies insurance eligibility and processes referrals with insurance providers.
• Requests appointments from appropriate specialty providers.
• Maintains knowledge of referral resources, forms, and key contacts; updates as needed.
• Tracks and processes referrals using the SOAP note system and related modules.
• Performs routine administrative tasks to support referral coordination and compliance with laws.
