Job summary

PeopleOne Health Medical Group PA is seeking a Patient Navigator to guide patients through the healthcare system, focusing on coordinating medical referrals and ensuring timely access to specialty care. You will work with clinical staff and external organizations to improve continuity of care and outcomes.

Responsibilities include processing referrals, communicating with providers, tracking status, educating patients on insurance and follow-up, and maintaining accurate EMR records.

Qualifications

• High School Diploma required; Bachelor’s preferred in a related field.
• Certification in patient navigation or case management is a plus.
• Experience using EHR and patient management software.

Responsibilities

• Coordinate and process patient referrals to specialty providers, imaging services and other resources.
• Communicate with referring and receiving providers to ensure information sharing and scheduling.
• Track referral status and follow up with patients and providers.
• Educate patients about the referral process, insurance requirements, and follow-up.
• Serve as liaison between patients and providers for scheduling, transportation, and language needs.
• Document all referral activities in the EMR system.
