Job summary

Trilogy Behavioral HealthCare in Chicago seeks a Reentry Team Leader to supervise a case management team serving individuals with severe mental illness transitioning from correctional facilities to community housing. You will oversee intake, assessment, and coordination with IDOC and CCSO to ensure referrals and access to housing and services.

The role blends clinical supervision, program operations, and grant compliance with a trauma-informed, harm-reduction approach.

Qualifications

• Master’s Degree in Social Work, Psychology, or Counseling, or a Bachelor's degree with 5 years of relevant experience.
• LCSW or LCPC preferred.
• Minimum two years’ experience with mental health and/or substance use diagnoses.
• Leadership experience highly preferred.
• IM+CANS certification preferred.
• Valid Illinois driver’s license with daily access to a well-maintained vehicle and $100,000/$300,000 liability insurance.
• Experience with low income housing, housing navigation, or homeless services preferred.

Responsibilities

• Conduct assessments and treatment planning for newly intaked clients and set priorities with the team.
• Split time between direct service, administrative, and supervisory duties.
• Provide direct clinical supervision and training to Housing Case Managers and Recovery Counselors.
• Provide trauma-informed, strengths-based, client-centered services to clients with severe mental illness.
• Maintain relationships with IDOC and CCSO to ensure smooth referrals.
• Manage employee workflows and scheduling to cover discharges and ongoing support.
• Communicate with internal teams and external partners; report trends and updates to Supervisor.
• Support Clinical Director with grant compliance and monthly/quarterly reports.
• Lead clinical documentation (IM+CANS, LOCUS) and transfer docs.
• Provide on-call coverage and crisis intervention as needed.
• Assist with funding-source expectations.
• Perform other related duties and/or projects as assigned.
