TRILOGY BEHAVIORAL HEALTHCARE in Chicago, IL seeks a Recovery Counselor to assess, plan, and deliver client-centered services for individuals with severe mental illness transitioning back into the community. The role emphasizes trauma-informed, harm-reduction approaches and strong coordination with court and community resources.

Responsibilities include comprehensive assessments, recovery planning, and providing home, community, and office-based support.

Qualifications

• Bachelor's degree in counseling or related human service field or equivalent.
• Licensed professional credentials are considered.
• CRSS certification or IAODAPCA credential preferred.

Responsibilities

• Assess client’s personal, medical, emotional, social, and environmental situation.
• Develop individual recovery plans with clients and support systems.
• Provide community-based treatment and psychoeducation.
• Coordinate referrals to housing, entitlements, and supportive services.
• Document progress notes following agency standards.
• Assist with court and probation-related navigation as needed.
• Collaborate with CCSO and Trilogy team on referrals and outreach.
• Support evening shift coverage for discharge-related needs.
