Job summary

Old Colony YMCA is seeking a Reentry Services Case Manager in Framingham, MA to support clients returning from incarceration through the Ralph Gants Reentry Services Program (RGRSP). You will engage with individuals, build rapport, and coordinate services (education, housing, career counseling) within a multidisciplinary team.

The role requires two years of justice-involved experience, a valid driver’s license, and strong communication skills, with evening availability and some travel expected.

Qualifications

• Bachelor’s degree or lived-in experience and intent to continue toward bachelor’s degree.
• Minimum of two years’ experience working with justice-involved individuals.
• Must have a valid driver’s license and ability to pass drug testing.
• Strong oral, written, and presentation skills.
• Proficiency with MS Office: Word, Excel, Outlook and Teams.

Responsibilities

• Publicize RGRSP services and enroll clients in CJSC programs.
• Develop rapport with individuals returning to the community after incarceration.
• Maintain monthly contact for 18 months with participants.
• Create case files with demographics, assessments and reentry plans.
• Manage caseload and coordinate with CJSC Public Benefits Case Manager.
