Job summary

Action Wellness is hiring a Prison Linkage Program Case Manager to deliver comprehensive case management to individuals currently incarcerated in, or recently released from, the Philadelphia County Prison system. The role focuses on reducing reoffending, improving health outcomes, and fostering long‑term stability.

The position requires a Bachelor’s degree in Social Work or related field and 2–3 years of case management experience with justice-involved individuals.

Qualifications

• Bachelor’s degree in Social Work, Human Services, Nursing, or a related field required.
• Minimum of two to three years’ experience in case management, social services, or healthcare.
• Experience working with justice-involved individuals, including those incarcerated or recently released.
• Experience in care coordination, client advocacy, and resource navigation within diverse communities.
• Familiarity with correctional systems, probation/parole processes, and community-based service providers.

Responsibilities

• Conduct in-person needs assessments to identify client strengths, challenges, and service requirements.
• Manage incoming referrals from various sources to connect incarcerated individuals with services.
• Collect and update medical and demographic information with informed consent.
• Prepare and update service care plans every six months.
• Coordinate and oversee service delivery while assisting clients with benefits and entitlements.
• Deliver case management and advocacy services to HIV-positive clients, including in-person visits.
