Job summary

NORTHEAST COUNSELING SERVICES is seeking a Prison Reintegration Case Manager to help justice-involved individuals access recovery services, housing, employment, and community supports. You will work in community settings, meeting clients where they live and participate in daily life.

The role emphasizes collaboration with social services, mental health resources, and reintegration supports. FBI, PA criminal, and child abuse clearances are required.

Qualifications

• Bachelor's degree in social services is required or related field.
• Knowledge of mental health, substance use, and social service systems.
• Behavioral health experience preferred.

Responsibilities

• Connect individuals with services and supports to achieve recovery, stability, and independence.
• Provide case management in community settings, meeting clients where they live, work, and participate in their communities.
• Collaborate with partner agencies, courts, and support networks to remove barriers to reintegration.
• Support justice-involved individuals transitioning back into the community after incarceration.

Education

Bachelor's Degree in Social Work, Psychology, or a related field
