Job summary

TASC, Inc. seeks a full-time Case Manager to support clients across Illinois, coordinating behavioral health services, benefits enrollment, and community resources.

You will assist individuals with service plans, conduct intake packets, and collaborate with partners to meet benchmarks while maintaining confidentiality.

Qualifications

• High school diploma or GED required.
• Valid driver's license.
• CADC certification within two years may be required based on contractual obligations.
• Pass an IDOC Background check.
• Proficiency in Microsoft Office suite.
• Experience leading group facilitation.
• Knowledge of ASAM placement criteria and DSM-V related to substance disorders.
• Comfort working inside a correctional setting with individuals.
• Strong time management and ability to meet deadlines.
• Excellent verbal and written communication skills.

Responsibilities

• Assist individuals in achieving service plan goals through direct services and coordination of behavioral health, public agencies, and community resources.
• Assess, evaluate, facilitate, and monitor progress against contractual benchmarks.
• Complete intake packets for program enrollment and participation.
• Provide referrals and crisis intervention for needs identified upon release from prison.
• Interview individuals, review records, and confer with professionals to identify reentry needs.
• Collaborate with internal and external partners, participate in staff meetings and supervision, and participate in integrated staffing with clients.
• Identify and refer individuals to initial treatment and behavioral health services.
• Complete and submit internal and external program reports.
• Maintain all required documentation for individuals and programs.
• Assist clients with enrollment in public benefit systems and obtaining vital records.
