Responsibilities

• Assess and monitor progress per service plan and parole conditions.
• Complete intake packets for enrollment and participation.
• Provide referrals and crisis intervention for identified needs.
• Interview clients, review records, and consult with professionals.
• Collaborate with partners and participate in staff meetings.
• Identify and refer clients with behavioral health needs.
• Prepare internal and external program reports.
• Document screenings, enrollments, and progress notes.
• Ensure program compliance in databases and records.
• Support enrollment in public benefits and vital records.
