Job summary

Lifeline Connections is a community-based behavioral health organization offering confidential care for substance use and mental health conditions in the Pacific Northwest. We seek a Re-entry Program Coordinator non-exempt to support participants transitioning from custody with data entry, financial tracking, and transition planning.

Qualifications

• Bachelor's degree in human sciences or related field preferred.
• High school diploma or equivalent with relevant college coursework required.
• One year experience working with adults or youth with co-occurring mental illness and substance use dependency.
• Familiarity with RCW 71.05 and WAC 246-341 regulations.

Responsibilities

• Coordinate with the RCS Program Administrator to obtain referral packets and designation letters.
• Coordinate with Department of Corrections to attend pre-release meetings and plan care transitions.
• Create and maintain a budget, access support funding, and ensure program requirements are followed.
• Maintain complete and up-to-date clinical files per WAC 246-341.
• Review WACs/RCWs and Federal regulations to ensure compliant therapeutic services.
• Support and implement agency policies and procedures.
• Provide referrals to mental health, substance use, or other treatment services as needed.
• Facilitate monthly RCSP meetings with community partners.
• Maintain accurate financial records for purchases using supportive funding.
• Maintain monthly service summary report.
• Represent the Agency in community meetings with supervisor approval.
• Other duties as assigned
