Job summary

Lifeline Connections in the Pacific Northwest provides community-based behavioral health services. The Mental Health Case Manager will perform assessments, develop and monitor Re-Entry Care Plans, and coordinate services to support recovery and community reintegration.

The role emphasizes trauma-informed, person-centered care, collaboration with a multidisciplinary team, and adherence to regulatory requirements to ensure high-quality client outcomes.

Responsibilities

• Conduct comprehensive assessments including whole-person and Health-Related Social Needs (HRSN) assessments.
• Develop, implement, and monitor Re-Entry Care Plans that are strength-based and participant-centered.
• Regularly evaluate participant progress and adjust care plans to support recovery and community integration.
• Provide targeted re-entry case management services in line with Medicaid Re-Entry Demonstration Initiative.
• Coordinate access to behavioral health treatment, medical care, housing resources, transportation, employment supports, and benefits.
• Facilitate referrals and ensure continuity of care across providers and systems.
• Collaborate with interdisciplinary treatment teams and community stakeholders.
• Maintain timely, accurate documentation in accordance with applicable laws and agency policies.
• Engage with community agencies and represent the agency professionally in meetings and forums.
