Job summary

StartupNWA is seeking a Reentry Program Specialist to support justice-involved individuals and veterans through a 16-week paid Reentry and Transition Employment Opportunity program. You will provide intensive case management, teach essential life and job skills, and help participants secure gainful employment.

Qualifications

• Minimum of 3+ years experience in public service or social services.
• Bachelor's degree preferred.
• Knowledge of the criminal justice system or veteran services.
• Ability to travel regionally.
• HIPAA/privacy awareness and compliance.

Responsibilities

• Provide intensive case management and assist participants in identifying social, vocational, and life skills goals.
• Design and facilitate educational and training programs
• Facilitate intake, complete needs assessment, and prepare Individual Service Plans
• Provide information and referral services and maintain professional boundaries
• Develop relationships with community service providers and agencies
• Maintain relationships with family members, funders, and internal/external contacts
• Ensure HIPAA compliance and CARF accreditation requirements
• Coordinate discharge planning and transfers to other agencies
• Coordinate staff in-service training and mentorship
• Complete daily and weekly case notes and progress reports
• Provide crisis intervention and support for reintegration
• Perform other related duties as required
