Job summary

Washington Corrections Center in Shelton, WA is seeking a non-permanent Corrections Specialist 2, Reentry Care Coordinator. This role will coordinate reentry care and link incarcerated individuals to health services, working closely with the Health Services Reentry Team to remove barriers to continuity of care.

The position requires strong interpersonal and communication skills, with collaboration across clinicians, social workers, and community partners to ensure a smooth transition from prison

Qualifications

• Three (3) years of experience with care coordination, case management, discharge planning, or other similar work in a healthcare setting.
• Proficiency using Microsoft Office Software (Word, Excel, Outlook) and SharePoint.
• Excellent written and verbal communication skills.
• Three years of professional experience in adult corrections, social services or closely related field.
• Experience with case management for physical and/or behavioral health needs.
• Knowledge of community resources.

Responsibilities

• Develop, coordinate and implement healthcare reentry planning for patients releasing from prison.
• Reduce access to care barriers and improve care coordination for patients releasing from prison.
• Serve as patient liaison and advocate to achieve quality patient outcomes.
• Participate in Clinic/Facility/Statewide Coordinated Quality Improvement Meetings, Projects and Programs.
