The City of New York, Kings County District Attorney’s Office, seeks an Outreach Specialist in the Community Resources & Re-Entry Bureau under the KCRTF Grant to support reentry participants with intake, case management, and referrals.

The role involves data tracking, outreach at facilities and events, coordination with staff, and travel to NYS correctional facilities to present information and plan reentry sessions.

Qualificações

• Bachelor’s degree or higher in a human services field is preferred.
• Associate degree with at least two years of social service experience acceptable.
• Experience with reentry services and providing case management is a plus.

Responsabilidades

• Facilitate intakes with prospective REENTRY participants.
• Input intake data into the tracking sheet.
• Maintain intake case notes.
• Case conference with REENTRY staff to refer participants to services.
• Provide short-term individualized case management (housing, healthcare, employment).
• Travel to NYS correctional facilities to conduct information sessions and planning sessions.
• Track and maintain data related to outreach and reentry planning.
