• Manage relationships within detention to coordinate service plans and discharge plans.
• Establish relationships with community providers to aid youth after release.
• Engage youth to identify strengths, needs, and interests.
• Coordinate with DYFJ staff, partner health services, and DOE staff for continuity.
• Prepare and maintain reports using forms, service plans, discharge plans, youth surveys, and databases.
