Job summary

The Fedcap Group is seeking a Reentry Mentor for Fedcap NYC's Transition Planning & Transportation program. You will engage with individuals housed in Rikers Island facilities, guiding them to services, coordinating scheduling, and supporting pre-release connections.

Key responsibilities include outreach, documentation, and collaboration with DOC staff to ensure smooth transitions. A high school diploma is required with lived incarceration experience preferred for this role.

Qualifications

  • Must have a High School diploma or equivalent; an associate degree is preferred.
  • Minimum 1 year of outreach, peer navigation, case support, or community engagement experience.
  • Lived experience of incarceration is required; experience with justice-impacted individuals is valued.

Responsibilities

  • Facilitate orientations in housing units to market program services to Persons in Custody (PICs).
  • Coordinate with Department of Corrections staff for unit assignments and outreach weekly.
  • Track PICs attendance in workshops and transition planning sessions.
  • Follow up with participants to provide updates on housing referrals, documentation needs, or deliver court letters and other materials promptly.
  • Accompany and support participants during community transitions and ensure follow-through on scheduled connections.
  • Maintain timely documentation (≥80%) of sessions, outreach, and follow-up in FedcapCARES and DOC-tracking systems.
