Job summary

Central Clinic Behavioral Health is seeking a proactive Program Coordinator for Reentry Support Services in Cincinnati, OH. You will manage crisis response, develop treatment plans, and coordinate care with courts and community partners to support clients reentering the community.

The role requires a Master's degree in social work or counseling and an Ohio license, with experience in legal-system-involved populations and co-occurring disorders.

Qualifications

• Master's Degree in social work, counseling or other related field.
• Independent Ohio license in either social work or counseling.
• Experience working with individuals involved in the legal system and working on a multi-disciplinary team.
• Knowledge of co-occurring mental health and substance use disorders.

Responsibilities

• Oversee crisis response and high-acuity service of operations, including clinical decision making, risk mitigation, safety practices, and continuity of care.
• Develop, implement, and monitor treatment plans for everyone on caseload; coordinates and consults with others to develop treatment plans.
• Complete diagnostic assessments and provide individual therapy in correctional settings and office-based setting.
• Provide clinical supervision to reentry staff (Case Manager).
• Coordinate referrals and care planning while supporting communication among courts and community partners.
• Develop and maintain pre-release plans in collaboration with clients, jail staff, courts, probation, treatment providers and other partners.
