Job summary

Central Clinic Behavioral Health is looking for a full-time Case Manager for Reentry Support Services in Cincinnati, OH. You will provide critical case management to justice-involved clients, facilitating their access to the necessary behavioral and community services.

This position requires a Bachelor's degree and at least 2 years of case management experience. A valid Driver's License and knowledge of behavioral health treatment are also needed.

Qualifications

• Minimum of 2 years of Case Management experience.
• Dependent license in social work, counseling or related field.
• Valid Driver's License and Auto Insurance.

Responsibilities

• Provide direct case management services.
• Assist justice-involved clients navigate treatment services.
• Work collaboratively with other agencies involved in client care.
• Maintain/update necessary client documents.
• Facilitate warm handoffs to treatment providers.
