Job summary

Central Clinic Behavioral Health is looking for a full-time Case Manager for Reentry Support Services in Cincinnati, OH. You will provide critical case management to justice-involved clients, facilitating their access to the necessary behavioral and community services.

This position requires a Bachelor's degree and at least 2 years of case management experience. A valid Driver's License and knowledge of behavioral health treatment are also needed.

Qualifications

• Dependent license in social work, counseling or related field.
• Valid Driver's License and Auto Insurance.

Position Summary

• Provide direct case management services, meeting annual delivery unit expectations, including treatment service planning, assistance in daily living, support services, crisis intervention, linkage/referral advocacy and support of client's self-determination.
• Maintain/update necessary client documents, records and reports.
• Work collaboratively with other agencies involved in client care.
• Complete necessary documentation and paperwork according to agency policy.
• Initiage engagement and coordinate behavioral health assessments upon referral from non-specialty court dockets, correctional settings, or partner agencies.
• Assist justice-involved clients navigate treatment and community-based services before and after release from incarceration.
• Provide care coordination and reentry support to clients through connecting with treatment, medical care, housing, transportation, benefits, and other community services.
• Facilitate warm handoffs to treatment providers and community-based support through pre-release planning and transition from incarceration.

Adult Services supports adults who struggle with severe mental illness. Each person receives individualized services including counseling, case management, group therapy, and prescription-drug management. Many individuals served are connected to our high-impact programs, such as Culturally Responsive Services, Transgender Wellness Program, and the LGBTQ+ Center.

CDC Behavioral Health Services is located in Butler and Clermont counties and provides mental health and substance use assessment and treatment services to children, families and adults. One specialty program is The Family Healing Center, which serves children and their families in Butler and Clermont counties through trauma-focused therapeutic visitation, successfully helping families build the skills necessary to be considered for reunification.

The Child & Family Treatment Center's primary objective is to help children be emotionally and developmentally prepared to succeed in school and home environments. This is achieved through our evidence-based practices, our direct-service programs, and our school-based prevention services.
