452 Mental Health and Substance Abuse Service is seeking a qualified professional to link individuals leaving correctional facilities to mental health and substance abuse services, housing, and recovery support in the community. The role involves managing a caseload and collaborating with correctional staff as part of an interdisciplinary team.

Candidates will screen and refer individuals to various state-wide reentry programs, working with social benefits and educational assistance entities.

Qualifications

• Master's degree in a behavioral health field or closely related behavioral science field with 1 year of experience in professional case management behavioral health services.
• Bachelor's degree in a behavioral health field with 2 years of professional experience in professional case management behavioral health services.

Responsabilités

• Link individuals leaving correctional facilities to mental health and substance abuse services, housing, and recovery support in the community.
• Manage a caseload and work with an interdisciplinary team.
