Job summary

HealthRIGHT 360, a network of programs across California, seeks a Case Management professional to support the STOP Area Six initiative in the San Diego region. You will manage a caseload of CDCR clients, develop CMPs, and coordinate transitions from custody to community-based care.

You will also arrange placements, monitor progress, document thoroughly in electronic systems, and collaborate with parole and community providers to connect clients with needed services.

Qualifications

• Bachelor’s degree in social work, psychology, or related field.
• Minimum 2 years’ experience working with criminal justice population.
• SUD treatment/recovery program experience.
• First Aid and CPR certified within 30 days of employment.
• Possess a valid CA driver’s license.
• Familiarity with community corrections systems and current practices.
• Ability to enter data into various electronic systems with accuracy.
• Computer literacy and proficiency in Microsoft Office.

Responsibilities

• Maintains an average assigned caseload of CDCR clients and supports treatment goals.
• Develops and monitors Case Management Plans (CMP) and links to internal/external resources.
• Maintains progress notes and exit/aftercare plans; prepares referrals to community programs.
• Arranges placements, transportation, and matches inmates to appropriate care levels.
• Completes timely documentation in electronic systems and coordinates with supervisory staff.
• And perform other duties as assigned.
