Job summary

La Familia in Hayward, CA is seeking a Clinical Case Manager to provide trauma-informed, clinically oriented case management to adults transitioning from incarceration back into the community.

You will conduct assessments, develop recovery‑focused plans, offer field-based services and telehealth, and collaborate with probation, housing partners, and social services to support housing, health, and employment.

Qualifications

• BA degree with minimum two years of experience in behavioral health or MA in related clinical field.
• Experience working with justice‑involved populations or reentry services strongly preferred.
• Bilingual English/Spanish is highly desirable.
• Strong clinical documentation and assessment skills are essential.
• Proficient in MS Office and Google Workspace; valid CA driver’s license required.

Responsibilities

• Conduct comprehensive clinical and reentry assessments focusing on mental health, housing, and psychosocial needs.
• Perform and document MSEs and safety risk assessments.
• Develop individualized recovery‑oriented service plans for community reintegration.
• Provide intensive field-based case management and telehealth as needed.
• Coordinate with probation/parole, housing partners, and behavioral health providers to ensure coordinated care.
