Amity Foundation in Los Angeles, CA is seeking compassionate Advocates (Case Managers) to join our community and support participants in our therapeutic programs. This role focuses on intake, needs assessment, CMP development, progress monitoring, and referrals to external services within a supportive, recovery-focused environment.

As an Advocate, you will maintain comprehensive case records, coordinate with CDCR liaisons, and develop discharge plans to assist participants’ successful re-entry.

Formación

• Bachelor’s degree in Social Sciences or related field or high school diploma with 2 years’ criminal justice-related experience.
• Operational knowledge of MS Word, Excel, and Access.
• Excellent interpersonal and communication skills.

Responsabilidades

• Provide program components and services to THP participants.
• Conduct intake and needs assessments for caseload.
• Develop and review Case Management Plans (CMP).
• Monitor participant progress and coordinate with CDCR staff.
• Document progress and services in participants’ files.
• Refer participants to outside agencies as needed.
• Develop discharge and post-completion plans.
• Maintain accurate case records and statistical information.
• Initial orientation and sign-in upon arrival; document assessment.
