Job summary

A community health organization is seeking a Medical Case Worker II to support Medi-Cal–eligible adults involved in the justice system in Los Angeles. This role is responsible for providing release planning services and coordinating health services for individuals transitioning from incarceration. The ideal candidate will have a bachelor’s degree and experience in casework, as well as the ability to work in correctional health facilities. Additional qualifications include successful completion of applicable clearances and good judgment in stressful situations.

Qualifications

• Experience with forensic history, mental illness, or dual diagnosis is preferred.
• Must clear the LA County Live Scan process.
• Clear Health Clearance required.

Responsibilities

• Provides release planning for clients involved in the justice system.
• Coordinates client release with community programs.
• Documents client encounters in medical charts.

Skills

Ability to follow oral and written instructions

Sound judgement in stressful environments

Ability to lift up to 10 pounds

Education

Bachelor’s degree from an accredited college
