Job summary

Weld County is seeking a professional case manager to support adult offenders in a Work Release facility. You will assess needs, develop reentry plans, and coordinate resources to promote community reintegration.

Responsibilities include data documentation, client instruction on life skills, collaboration with security personnel, and maintaining records. A Bachelor's degree and 2 years in related field are required; bilingual skills are desirable.

Qualifications

• Bachelor's degree in criminal justice, social work, social sciences, sociology, psychology, counseling or similarly related field.
• 2 years of professional experience in a correctional counseling/case management services position or human services related environment.
• Bilingual English/Spanish skills are desirable and helpful.
• Strong communication, organization and problem-solving abilities.

Responsibilities

• Assess clients, develop reentry plans based on risk and needs.
• Maintain and analyze data related to service delivery and outcomes.
• Coordinate community resources and establish linkages with external agencies.
• Collaborate with security and administrative staff to support facility operations.
• Deliver individual and group instruction on cognitive skills, housing, employment, relapse prevention and life skills.
• Oversee documentation, and manage medication dispensation as needed.
• Input data and generate reports to evaluate client compliance and outcomes.
• Represent the program at justice system committees and meetings.
